Remember: Don't open the door Immunization Policy It is SAD #46 School Board Policy, along with State of Maine Law, that all SAD #46 students in grades K-12 must have immunizations. The following immunizations are needed: Five (5) DPT shots, four (4) Oral Polio's and (2) MMR shots and at least one (1) Varicella (chicken pox). If your child has had chicken pox you need to provide a signed letter stating the month and year in which he/she had the disease. If you do not show proof of these immunizations within 90 days of being notified, your child (ren) will be dismissed from school until immunizations are brought up to date. For any exemptions: Medical or Religious/Philosophical reason(s) - A new signed/dated letter is required each school year whether it is medical or philosophical. You may either write a new letter each year or fill out this form and re-sign it each year. Medication Policy/Paperwork This is to notify all parents that there is a new form for medication that will be given at school. The new form will need a doctor’s signature for any medication that will be given or available to give for more than 15 days at school. A doctor’s signature is NOT required for Tylenol, ibuprofen, cough/cold medicine, or benadryl for allergies. Also please be reminded that the school does not provide cough drops to students. Parents must send these in with a note that gives permission to have them. If your child has asthma and carries his/her own inhaler the medication form must still be filled out. As always, we encourage medication to be given outside of school times. However, we realize this is not always possible. Please help us by completing the necessary paperwork when your child does need medicine at school. If you have any questions about the new form or any other concerns please call Mrs. Poland, the school nurse at 924-7671, ext. 120. Thank you! Dental Health Weekly fluoride rinse starts in October and continues through out the school year for primary students. Fluoride is important to promote healthy teeth and fight decay. It is especially important to have this supplemented when you do not have fluoride added to your water. Students will also be given a new toothbrush some time during the year. Screenings There are several screenings that are completed throughout a child's academic career. They include vision, hearing, height, weight, and scoliosis. When these screenings are completed parents/guardians will be notified of the results. If you ever have a concern or question about any of these or would like to request a screening, please contact me.
